One Payment Plan:

By selecting this plan, I understand that my payment for the current session is paid in full by September 10.

Two Payment Plan:

By selecting this plan, I understand that my payment for the current session is payable in two (2) payments, the first paid no later than September 10 and the second paid by January 10.  There will be a $20 late fee on payments not received within 10 days of due date.

Seven Payment Plan:

By selecting this plan, I understand that my payment for the current session is payable in equal monthly payments plus 5% commencing September and including April 1.  No payment is made in December and May.  There will be a $5 late fee on tuition not received by the tenth of the month.

Fees

In submitting the above registration form to Jo Bonds Dance Studios, Inc., I agree to pay the required charges as specified above, and I agree that students will be responsible for adhering to the rules of the studio as presented by the studio.  Tuition is non-refundable.  There is a $30 fee on all returned checks.
